Enlarging the chance that clarification-question comments would be modular
Even threaded, asking two or more clarification questions in one comment can be frustrating (and a miscommunication bias) for an OP, especially if the OP doesn't speak English as a native language and/or if the OP is feeling "haunted".
Since the community can't really control how many clarification questions are being written in one comment, I suggest to consider these options:
- Printing a small hint like We suggest to ask just one clarification question per thread
- Asking the commenter
Is your comment a question?
and if the commenter hits "yes" than a the question would be inserted under aWhat is your clarification question?
title with a button labeledAsk another clarification question
appearing under the question for a case of need, similar to "add another file" button after uploading an image to a local file tree

While I somewhat agree with your concerns I don't think providing some additional system that checks comments is a good idea (second suggestions). Especially across all communities.
Here is my thoughts:

Such hint would annoy some users, myself included. The less system intrudes to communication between users than better. I'm tired of "clever" things that "knows" better what my intents are.

Communities are totally different. What seems inappropriate for one can be absolutely normal for another. Moreover different rules can be applied to different categories within one community.

Even if such system is implemented nothing can stop a user from writing a comment with many Qs.
It seems to me that you're trying to solve problem before it's even occurred. Maybe it would be better to return to this Q when it's right time?
